Security and Privacy Challenges in the Internet of Medical Things (IoMT): A Systematic Literature Review   Abstract Internet of Medical Things (IoMT) stands out as a revolutionary concept in healthcare, facilitating timely patient monitoring, remote diagnostics, intelligent decision-making within the clinical environment, and customized care via integrated medical equipment, wearable devices, cloud infrastructure, and AI systems. The widespread adoption of IoMT has transformed healthcare access, efficiency, and patient care, making it more accessible and efficient. The massive amount of sensitive medical information that needs to be collected, transmitted, and stored, however, has also raised significant security and privacy concerns, jeopardizing the reliability, confidentiality, and trustworthiness of IoMT-enabled healthcare systems. Malware, ransomware, denial-of-service attacks, man-in-the-middle attacks, insider threats, and unauthorized access are among the many forms of cyberattacks that continue to highlight significant vulnerabilities within healthcare infrastructures, potentially endangering patient safety and regulatory compliance. The objective of this study is to perform a systematic and comprehensive review of the already published work on security and privacy issues for IoMT and to find out the existing countermeasures, emerging technologies and future research possibilities. The methodology used was Systematic Literature Review (SLR) to guarantee a transparent, rigorous and reproducible synthesis of the existing knowledge. Using predetermined inclusion and exclusion criteria, ten high quality and influential studies published from 2018 to 2025 were chosen from the top scientific publishers and analysed qualitatively. It reviewed the significant security threats, privacy issues, present security solutions in an existing IoMT scenario, upcoming technological developments and unmet research challenges in IoMT environments. The results show that the most important security concerns are authentication vulnerabilities, access control, insecure communication channels, malware, ransomware and device vulnerabilities, while patient data breach, privacy of identities, privacy of clouds, and consent management are the most important privacy concerns. Lightweight cryptography, blockchain technology, artificial intelligence, machine learning, deep learning, federated learning, edge computing, and intrusion detection systems and Zero Trust Architecture are now combined into existing countermeasures to improve cybersecurity and privacy protection. However, there are a number of challenges in securing large-scale IoMT ecosystems, including: resource constrained devices, interoperability, regulatory compliance, explainable A.I., and resilience to changing cyber threats. The main contribution of this study is that it offers a comprehensive overview of recent works in IoMT security and privacy by systematically analyzing key works, uncovering common research trends, summarizing the challenges which have yet to be resolved, and suggesting future research directions. This paper is different from the previous on the technologies or individual security aspects as it considers a comprehensive approach that covers the topics of cybersecurity, privacy preservation, intelligent defense mechanisms, and emerging technologies in a unified analytical framework. The results offer relevant information for researchers, healthcare institutes, policy makers, and medical device manufacturers to design safe, privacy-preserving, and trustworthy IoMT-based healthcare systems. In summary, to foster sustainable and secure IoMT environments, it is essential to implement intelligent cybersecurity solutions, adopt privacy-by-design principles, establish standardized security frameworks, and promote interdisciplinary collaboration to facilitate the safe integration of next-generation digital healthcare technologies.
